Our clinics accommodate referrals directly from community physicians and through the UW Medicine Contact Center at 206.520.5000.

If you are a provider and would like to refer your patient to UWGI, please complete the External Referral Form on the Patient Care page and fax it to 206.598.4303.

Please note that submission of a referral does not guarantee referral acceptance.
